Choice Healthcare is enrolled with Medicare and accepts assignment. Where Medicaid is primary, we prepare and submit the prior authorization request — the FA-1 or FA-1B in Arizona, and the DME prior authorization request through the provider portal in Nevada.

What the patient pays
Coverage and patient responsibility are set by the plan, not by us, and depend on the documentation submitted with the claim. Under Medicare Part B a deductible and 20% coinsurance typically apply.
We tell patients what their share is expected to be before anything is delivered, and if an item isn't covered we say so in writing first. Nobody finds out from a bill.
Prior authorization
Several categories — power mobility above Group 1, custom seating, scooters and some bracing — require authorization before delivery. We prepare and submit it, track the decision, and handle any denial or second review ourselves. How that works →
