Privacy & HIPAA Notice

How Choice Healthcare, Inc. handles information collected through this website, and how protected health information is handled once you become a patient.

Short version. This website collects only what you type into the contact form — your name, phone number, email, organization and message. We ask you not to send patient health information through it, because a web form and ordinary email are not secure channels. We use Google Analytics to see how the site is used; nothing you type into the form goes to it. We do not sell your information, and we do not share it with advertisers.

1. Two different kinds of information

There is a meaningful difference between information you send us through this website and the protected health information (PHI) we hold once you become a patient. They are governed differently and this notice addresses both.

Website information

When you submit the contact form we receive the name, callback number, email address, organization and message you entered, plus the date and time of the submission. Our web host also keeps standard server logs, which typically record IP address, browser type and the pages requested. We use this only to respond to you and to keep the site working and secure.

Protected health information

Once you are a Choice Healthcare patient, the clinical and billing information we hold about you — your diagnosis, the equipment ordered, your physician's records, your insurance information — is protected health information under the Health Insurance Portability and Accountability Act (HIPAA). We use and disclose it for treatment, payment and health care operations, and otherwise only as HIPAA permits or requires, or with your written authorization.

2. Please don't send patient details through this website

The contact form on this site is delivered by ordinary email. That is not a secure or HIPAA-compliant channel, and we will not treat it as one. Do not enter patient names, diagnoses, dates of service, insurance identifiers or any other clinical detail into it.

Give us a name and a callback number instead. We will call you and take the clinical information by telephone, secure fax at (888) 507-7136, or another channel appropriate to the information being shared. If you do send us PHI through the form anyway, we handle it under the same protections as any other PHI we hold — but the transmission itself was not secure, and we would rather you didn't.

3. Cookies and analytics

This site uses Google Analytics 4, delivered through Google Tag Manager, to understand how people find and use it. It records things like which pages were viewed, how long a visit lasted, roughly where in the country the visit came from, what kind of device was used, and which link or search brought you here. It also records when someone taps one of the phone numbers on the site.

What it does not record. Nothing you type into the contact form is sent to Google — not your name, not your phone number, not your message. No patient information of any kind is transmitted to any analytics provider, because this site does not collect any. We do not run advertising or social media tracking pixels, and we do not use analytics to build advertising profiles of visitors.

Google Analytics sets cookies in your browser to tell repeat visits apart. You can block them through your browser settings, or install Google's official opt-out browser add-on, and the site will work exactly the same.

This site also loads a web font from Google Fonts, which means your browser makes a request to Google's servers to fetch the font file.

If we ever add advertising or remarketing tags, or begin collecting anything clinical through this website, we will update this page before doing so.

4. How we use what you send

  • To call or email you back about the referral or question you sent
  • To route your message to the right person — intake, billing, or a service technician
  • To keep a record of the request so that nothing gets dropped
  • To maintain the security and reliability of the website

We do not sell your information. We do not rent, trade, or provide it to advertisers or data brokers.

5. Who we share information with

We share information only where it is necessary to do the work, or where the law requires it:

  • Payers. Medicare, Medicaid, and commercial insurers, to obtain prior authorization and submit claims.
  • Treating practitioners. Physicians, physician assistants, nurse practitioners, and physical and occupational therapists involved in your care.
  • Business associates. Vendors that handle information on our behalf — billing services, our email and web hosting providers — each under a written agreement requiring them to protect it.
  • Manufacturers and service partners, where necessary to fulfill an order, a warranty claim, or a repair.
  • Government agencies, where required by law, including CMS and its contractors in connection with audits, inspections, and enrollment.

6. Your rights over your health information

Under HIPAA you generally have the right to:

  • Inspect and obtain a copy of your health records we maintain
  • Request a correction to information you believe is incorrect or incomplete
  • Receive an accounting of certain disclosures we have made
  • Request that we communicate with you at a specific address or phone number
  • Request a restriction on how we use or disclose your information
  • Receive a paper copy of our Notice of Privacy Practices on request
  • Be notified if there is a breach of your unsecured protected health information

To exercise any of these rights, contact us using the details below. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

7. Security

This website is served over HTTPS. We maintain administrative, physical and technical safeguards for the information we hold, and we limit access to staff who need it to do their job. No method of transmission or storage is perfectly secure, which is the reason for the request in section 2.

8. Children

This website is not directed at children and we do not knowingly collect information from children through it. We do supply equipment to pediatric patients; that information is handled as PHI under HIPAA, with a parent or legal guardian acting as the patient's personal representative.

9. Changes to this notice

We may update this page. The review date below reflects the most recent version. Material changes to our HIPAA privacy practices are made available to patients as HIPAA requires.
